Point is, we pay either way. We'd all pay less if the government took over as the national insurer. (Frankly, as little as I trust the government, I trust insurance companies even less.)
And, for heaven's sake, cover EVERYONE, American or not, legal or not. Not only is it the right thing to do, but it makes better business sense than excluding anyone.
(And don't get me started on the idiocy of mandates requiring everyone to have health insurance. Most States require you have car insurance, but, well, chances are that the idiot who sideswipes your car doesn't have any. Because it's too expensive. Because of uninsured drivers.)
